This service is not for emergencies. If you are unable to walk or experience severe trauma, fever, chills, worsening muscle weakness, bowel or bladder changes, or numbness in the groin, please seek in-person evaluation by calling 911 or presenting to the nearest emergency department.

Why we exist

Half of American adults live with musculoskeletal pain. Most MSK problems require simple treatments. Getting evaluated for these treatments, however, is anything but simple.

Seeing your primary care doctor can take weeks. Going to urgent care or the emergency room diverts important resource from the critically ill. Even when seen - it usually results in a non-urgent referral to primary care or an orthopedist. At Cobble Health, we are trying to make it easier and more affordable for you to access high quality musculoskeletal care.

Weeks of waiting

Primary care scheduling, specialist referrals, and authorization cycles delay simple answers for back, neck, and joint pain.

🧭

No clear front door

Telehealth platforms cover dermatology, mental health, and urgent care — but a person with back pain and a credit card has no MSK-specific option.

💸

Opaque prices

The same MRI can cost $400 or $4,000 depending on where you go. We route you to transparent cash-pay providers and tell you the price first.

How it works

Three steps. One flat fee. A plan within 24 hours.

1

Tell us about your pain

A guided questionnaire covers where it hurts, how it started, and what makes it better or worse — including a careful screen for warning signs that need urgent in-person care.

2

A physician reviews it

A board-certified physician licensed in your state personally reviews your evaluation. Every single one — no algorithm-only decisions.

3

Get your plan

You receive a written disposition: an X-ray or MRI order, an anti-inflammatory prescription, a physical therapy plan, a specialist referral — or clear instructions to seek in-person care.

Safety first. If your answers show signs of a serious condition — loss of bladder or bowel control, fever with spine pain, major trauma, progressive weakness — the evaluation stops and directs you to emergency or urgent in-person care instead of taking your money.
Services we Provide

What your evaluation can lead to

Some examples of what your reviewing physician may issue when clinically appropriate.

🩻

X-ray order

Targeted X-ray orders that obtain only the necessary diagnostic images without subjecting you to unnecessary radiation.

🧲

MRI order

When clinically indicated — with steering toward independent centers that charge a fraction of hospital prices.

💊

Medication Prescription

Anti-inflammatories and topical agents — with safety screening for your stomach and kidneys. Cobble Health does not prescribe controlled substances for any reason.

🏃

Physical Therapy Plan

Cobble Health was founded by orthopedic surgeons. We can provide not just a physical therapy prescription, but a tailored plan that is likely to improve your symptoms.

🩺

Specialist referral

Direct referral to physiatry or a surgical specialist when your evaluation warrants it.

🚨

Escalation guidance

If anything suggests a serious condition, you get explicit instructions for same-day in-person or emergency care.

Transparent pricing

You'll know every price before you pay it

The evaluation is $79, flat. Anything we recommend afterward is priced separately, in writing, at transparent cash rates — and you choose whether to proceed.

An honest word about insurance

What your insurance will — and won't — do with our orders

We'd rather set expectations clearly than oversell. What you can do and where you can take the orders we provide depends on your plan type.

PPO, EPO, or high-deductible plan? Most members

You don't need a primary-care referral to see specialists, obtain X-rays, or get PT in the first place. Our physician's imaging and PT orders can enter your insured pathway, subject to your plan's prior-authorization rules. MRI's often require prior-authorization and we can assist with this.

HMO or POS plan? Read this first

Your plan generally requires referrals to come from your own in-network primary care physician — a referral from us usually won't unlock in-network coverage. For you, our value is speed, expert triage, and routing to fairly-priced cash-pay imaging and PT, not an insurance shortcut. We tell you this up front because it's true.

Plain X-rays are routinely covered without prior authorization. MRI and CT almost always require prior authorization on commercial plans — often including documented failure of conservative treatment first. When that friction makes insurance impractical, our cash-pay imaging routes are usually faster and often cheaper than your hospital coinsurance.
Where we practice

Available in eight states at launch

Care is delivered by physicians licensed in your state. We verify your location before any clinical interaction begins — it's both the law and good medicine.

Built by Orthopedic Surgeons

Designed the way we'd want our own families triaged

Specialist-built clinical logic

The questionnaire and decision pathways were authored by fellowship-trained academic spine and orthopaedic surgeons — the same screening we use in clinic.

Clinically guided red-flag screening

Every evaluation screens for the conditions that can't wait: cauda equina syndrome, spinal infection, malignancy, and occult fracture. Warning signs trigger a hard stop and escalation — never a prescription.

A physician reads every one

AI helps structure your intake; it never decides your care. A licensed physician in your state reviews and signs every disposition.

Questions

Frequently asked

Is this a video visit?
No — and that's deliberate. You complete a structured written evaluation on your own time, and a physician reviews it the same day. For musculoskeletal triage, a careful structured history captures what matters; if your case needs a conversation or an in-person exam, your physician will say so in the disposition.
Who reviews my evaluation?
A physician licensed in your state, working with our affiliated medical practices. Your reviewer's name and credentials appear on your written plan.
Can you prescribe pain medication?
We prescribe non-controlled medications only — anti-inflammatories and topical agents. We do not prescribe opioids or other controlled substances, ever. This keeps our prescribing safe and lets us focus on what actually helps most musculoskeletal pain.
What if my answers suggest something serious?
The evaluation stops before payment and tells you exactly where to go — emergency department or same-day in-person care — and why. We will not charge you for an evaluation we cannot safely complete online.
Does insurance cover the $79 evaluation?
Not at launch — the evaluation is cash-pay. Many HSA/FSA cards work for medical evaluations. The orders we issue (imaging, PT) may be usable within your insurance depending on your plan type — see the insurance section above for an honest breakdown.
What happens after I get my plan?
Your written disposition includes everything you need to act on it: imaging orders you can schedule, prescriptions sent to your pharmacy, PT referral paperwork, or specialist referral details. Follow-up evaluations for the same problem will be available at a reduced price.
